Hi I would like to request to block my loss phone last November 27 2020. I try to contact my phone even sending messages hoping someone has a good heart found it and returning to me.But unfortunately they trying to drop and avoid it my call . I am writing and sending if you could help me with my concern. Di pa po kasi bayad sa home credit and ginagamit po sya ng pang online class ng mga anak ko . Thanks and have a good day/eve

Response to Request
Please be informed that the NTC has no capability to locate lost cellphone. However, you may file a request for the blocking of Lost/Stolen Mobile Phone/Unit at Consumer Welfare and Protection Division (CWPD) of the National Telecommunications Commission, BIR Road, East Triangle, Diliman, Quezon City. Our office is available from Monday-Friday 7:00 AM - 6:00 PM or call at 8-9213251 & 8-9267722 for further information.
However, due to this pandemic situation, we suggest you dial the contact details provided above before you proceed to the office and to bring the following requirements:
1. Duly accomplished Request for Blocking Form
2. Proof of Ownership, ANY of the following:
2.1 Copy of Official Receipt of the mobile phone
2.2 Box of the mobile phone with International Mobile Equipment
Identity (IMEI)
2.3 Copy of duly notarized Affidavit of Loss
Note: The Affidavit of Lost must indicate the IMEI of the mobile
phone and/or the SIM to be blocked.
3. Copy of valid Identification
3.1 Any government-issued ID or Passport
3.2 For students, school ID
3.3 For cases when ID is not available, Birth Certificate OR NBI
Clearance
Supporting Documents for Representative(s)
1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
