Hi I would like to request to generate my Vax cert. I was a frontliner in Agoo, La Union and soon be flying to Davao City with my husband. My 1st dose and 2nd dose was AstraZeneca dated March 19, 2021 and June 11, 2021 respectively. December 4, 2021 was my booster under Pfizer. Thank you!
