To whom it may concern: This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last december. However, your system always tells me that there is no second dose vaccination record found(12/27/2021). I have already submitted a copy of my vaccination card and my id to your system. I am really hoping you can help me.

July 18, 2022 Dear Ms. Girlie Visitacion, Greetings! Thank you for your request dated Jul 14, 2022 06:48:58 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for a copy of your Vaccination Certificate (VAXCert). Ms. Girlie De Guzman Visitacion PLEASE BE CORRECTLY INFORMED and RESPONSIBLE. OWWA has no mandate to issue Vaccination Certificates (VaxCert) but it is the Department of Health thru the Bureau of Quarantine or the Health Centers of Local Government Units who provide the Certificates. So address your request to the appropriate government agency. Moreover, you must use the EFOI portal properly. You registered thrice on July 14, 2022 at 6:27, 6:36, and 6:48 p.m. READ the eFOI and understand the procedures. Search for eFOI one-page manual in the internet so you can be more knowledgeable and act appropriately. e-FOI is not a call center. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing the OWWA- FOI Decision Makers at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter.
