You asked for VaxCert.
Response to Request
Your FOI request is approved.
VaxCertPH is the official digital vaccination certificate (DVC) for Filipinos and non-Filipinos vaccinated in the Philippines that may be used for international and domestic travel. It is free of charge, and complies with the WHO Digital Documentation of COVID-19 Certificates (DDCC) guidelines. Visit the VaxCertPH website at vaxcert.doh.gov.ph to generate your vaccination certificate. Please check the accuracy of your data based on your vaccination card (e.g. spelling of full name, correct dates, etc.) before submitting a request.
VaxCertPH has been updated with new features! All citizens who generated their certificates before February 7, 2022 are urged to request the new version of VaxCertPH in the portal, since the QR code of previously generated certificates will become outdated and unreadable. Regenerate your VaxCertPH if you already have it, or get your VaxCertPH 48 hours after your full vaccination.
VaxCertPH is currently in the soft launch phase, therefore vaccination records are still being uploaded by all LGUs to the VaxCert central database.
If your vaccination record is incomplete or unavailable, the details of your vaccination record may have been incorrectly encoded or have not been uploaded yet in the vaccine information management system (VIMS) by your vaccination center.
For concerns on no vaccination record, corrections or incomplete vaccination information in VaxCert, kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. Thank you
Your right to request a review
