Hi! I would like to follow up my Vax Cert Request with: 1. Referrence ID: 1707052 2. Ticket Number: 317771-1 To whom it may concern: This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last week 2 weeks. However, your system always declines it and always tells me that there is no vaccination record found. I have already submitted the copy of my vaccination card and my postal ID in your system, with a reference ID number of: 1707052, and a ticket number of 317771-1. I am really hoping you can help me.

Your Request You asked for VAXCERT. We regret to inform you that the Bulacan Agricultural State College has no possession of records regarding your vaccine certificate. However, you may coordinate with your LGU regarding your vaccine certificate or you may go to https://vaxcert.doh.gov.ph/ to check the status of your vaccine certificate. Your FOI request is approved. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Marilyn Alvarez at [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
