Hi Dears, Please assist to correct my name on my vaccination details. As per checking my first and second dose were updated without "Jr". But my booster or first additional shot is updated with "Jr." Details in my vaccine card has no Jr as well, but form filled up has JR. Last Name: Sumaylo First Name : William Middle Name : Baterna Suffix : Jr Birthdate : 03/14/1987 First Dose 06/17/2021 Sinovac - Pasig Second Dose 07/17/2021 Sinovac - Pasig Additional Dose 02/16/2022 Moderna - QC No Vax Cert was generated yet due to different names in site. One with Jr. And other one,without Jr.
