Hi! I would like to follow up my Vax Cert Request with:

To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since last week, November 28, 2021. 

However, your system always  tells me that there support specialist will respond and many days pass by and I did not receive any update from them. I just want my Birthdate Year to be changed from 1992 to 1993. I already submitted my vaccination card and UMID.

I am really hoping you can help me.

Please use this process if: Details of vaccination record have been incorrectly encoded, or You would like to update your information Click “Update record” to begin the process of updating. You will have to upload a photo of your vaccination card and government-issued ID and enter your email address and/or contact number. Your request will be forwarded to the LGU where you received your vaccination so they can validate and rectify through your email address or contact number once your request is completed and you may try requesting again your VaxCertPH through the website.

VaxCertPH is a portal and mobile app developed by the DICT for the Department of Health (DOH) for the issuance of digital vaccination certificates for vaccinated individuals in the country. Please be informed that the DICT does not handle management of personal information of vaccinees in the portal

Your right to request a review
