Hello, would like to request that my vaccination certificate include my booster shot for Medical Purposes. I am currently taking different medications for Asthma, GERD, Depression, and Anxiety and a complete vaccination card would be helpful in case of a medical emergency.

While our aim is to provide information whenever possible, in this instance this Office could not provide your request for issuance of vaccination certificate. However, you may wish to contact Department of Health (DOH) doh.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at their website.

Your right to request a review
