Hi! My name is Julius B. Daguman and i would like to know the status of birth certificate (JULIUS BALON DAGUMAN)
that i petitioned last july-august. It is really required for me to pass my birth certificate this month or next month for my US Visa. Thank you and i hope you'll grant my request as soon as possible.

I refer to my letter of Dec 07, 2020 03:57:53 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Update/Status of my birth certificate that has to be corrected.
In order for us to work further on your request, we need more information from you regarding your query. What do you mean by petition? Did you process petition for correction? May we know if you have already transmitted the Certificate of Finality?

That's the thing, i didn't received the certificate of finality yet but my cousin said he already sent the 1st endorsement (clerical error) to PSA and LCRO since july-august. that's why i wanted to follow it up or atleast know the status of it.
I also have the documents here that he gave me, the RA 9048 FORM and the 1st endorsement letter, plus the annotated copy of my birth certificate and lastly the certified true copy of the birth certificate. I can also the send pictures of it if u guys needed it. thank you and i hope u can reply as soon as possible.
