Hi! I would like to ask if do you have an updated arrival form and health declaration form that every airline passenger needs to comply/answer during their flight/arrival in the Philippines.

Your request You asked for Updated Arrival Form and Health Declaration Form. Response to your request While our aim is to provide information whenever possible, in this instance this Office could not provide your request for a copy of arrival and health declaration form. However, you may wish to contact Department of Health (DOH) for the health declaration form at doh.gov.ph and Manila International Airport Authority (MIAA) for the arrival card at miaa.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at their website. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the BI Commissioner, JAIME H.
