Dear Philip Mabute,
I refer to your letter of Nov 02, 2021 03:23:09 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for UP-Philippine General Hospital.
In order for us to work further on your request, you need to initially seek endorsement from the UP Manila Research and Ethics Board prior to the approval of the request. It can be accessed online through the UPMREB website (reb.upm.edu.ph) . There are forms to be filled up and instructions are given. Moreover, researchers need to register via online to the Research Grants Administration Office (RGAO). Once you have the clearance, the PGH Expanded Hospital Research Office (EHRO) will issue a permit for the researcher to proceed. You need to submit the permit here using the reply function on the eFOI portal at https://www.foi.gov.ph/requests/aglzfmVmb2ktcGhyHQsSB0NvbnRlbnQiEFVQTS0wNDAwNTg4NDM2NjkM, for request with ticket number #UPM-040058843669, to respond.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jan 31, 2022 01:38:13 AM, your request will be closed.

Dear Philip,
I refer to my letter of Nov 08, 2021 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for UP-Philippine General Hospital. In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Jan 31, 2022.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
