Request was tagged on- hold due to name discrepancy in my university system, encoded middle name "ESPIRITU" vs "E." in the Certificate of Live Birth. I visited the uni to resolve my concern, but due to my visit they learned that I am illegitimate so I was advised to file a petition to omit my Middle Name/ Middle Initial. Provided that my name is already annotated in the Certificate of Live Birth, I sent my second request for the release of the documents and presented my original Birth Certificate to the PUP Registrar but was asked again to submit another document which is the corrected Form137
Therefore I contacted my former High School regarding the correction of Form 137a, they reiterated that I graduated from them in 2015 and just got the Annotation in my Birth Certificate in 2019 hence, some of the signatories already left the school. I kindly asked for your assistance if there's a way to expedite or any remedy in order for them to release my TOR and Diploma. Alas, until now I don't have any proof of degree completion and these documents will be used for transitioning to another employer so I am hoping that this will be settled as soon as possible.
