Hello! I am Christian Cuevas, Sr. Admin. Asst I in the Office of the Provincial Prosecutor of Davao del Sur. I was appointed to government service on 18 January 2019 and in the same year, I applied for membership with your agency. Up until now, I have not received any notice regarding my UMID Card, thru my e-mail ([email protected]) or phone number (0905-485-0050). In this view, I would like to make a follow-through with my GSIS UMID Card. I am hoping that this be acted upon immediately. Thank you.

Good Day, Sir, as per varication made, you have only applied for membership with GSIS in 2019, the time you entered government service with DOJ, National Prosecution Office as Admin Asst. V. You need to enroll with UMID so that you can be issued UMID card. As of date, no record of enrollment has been entered yet. You may visit any GSIS office nearest you at your convenient time from Monday to Friday. Kindly bring along with you 2 government valid IDs as a requirement for the enrollment.
