Hi, I would like to inquire regarding the application of GSIS umid card. May I know too what is the process, do I need to have an online appointment or not? Thank you.

Good day. Relative to your query on UMID Enrollment, please see the following requirements below: 1. Go to your GSIS Handling Branch and enrol for your GSIS UMID Card (bring valid IDs). 2.Wait for an SMS from GSIS on when to pick up your GSIS UMID Card. 3. Prepare 2 valid government issued IDs to claim. In claiming the cards: Active: In person only. Bring an IDs. Pensioners: In person or through an Authorized Representative with the requirements to bring: Duly signed Authorization Letter OR notarized Special Power of Attorney (SPA); Photo or print-out of pensioner holding the Authorization Letter or SPA; Photocopy of the pensioner’s two (2) valid government issued IDs and its originals; and Photocopy of the authorized representative’s two (2) valid government-issued IDs and its originals. You may visit https://www.gsis.gov.ph/ginhawa-for-all/umid-card/ for more details. Thank you.
