Good day! I am Arly Khryss Joyce E. Obeso, an Aministrative Aide IV of the Office of the Provincial Prosecutor of Davao del Sur of the Department of Justice since October 2, 2018. In the same year, I applied membership in GSIS Agency. However, until this day, I have not received any notice regarding my GSIS UMID Card thru my email ([email protected]) or my phone number (09097278727). In view thereof, I would like to humbly request a follow-through with the said UMID Card. Hoping for your favorable consideration on this matter. Thank you.

Good Day, As per verification made, you have not yet enrolled with UMID. Please visit any GSIS office nearest you for the enrollment. Kindly bring 2 government valid IDs as a requirement for the enrollment. UMID card will be issued after the enrollment. Thank you
