Good day! May I request for my mobile device to be blocked the soonest possible? My phone was stolen Sunday morning and there are a lot of personal files there.
I have already sent an email to you but to no avail. I also tried calling all the hotlines provided but no one's answering and it just keep on ringing.
I have here with me the box with my phone's IMEI as well as the receipt. Here's the IMEI:

Response to Request:
Please be informed that the NTC has no capability to locate lost cellphone. However, you may file a request for the blocking of Lost/Stolen Mobile Phone/Unit at Consumer Welfare and Protection Division (CWPD) of the National Telecommunications Commission, BIR Road, East Triangle, Diliman, Quezon City. Our office is available from Monday-Friday 7:00AM - 6:00 PM or call at 8-9213251 & 8-9267722 for further information.
Furthermore, due to this current situation, we suggest you to dial first the contact details provided above before you proceed to the office and to bring the following requirements:
1. Duly accomplished Request for Blocking Form
2. Proof of Ownership, ANY of the following:
2.1 Copy of Official Receipt of the mobile phone
2.2 Box of the mobile phone with International Mobile Equipment
Identity (IMEI)
2.3 Copy of duly notarized Affidavit of Loss
Note: The Affidavit of Lost must indicate the IMEI of the mobile
phone and/or the SIM to be blocked.
3. Copy of valid Identification
3.1 Any government-issued ID or Passport
3.2 For students, school ID
3.3 For cases when ID is not available, Birth Certificate OR NBI

1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
