Your request You asked for modification of surname in your travel record from Anasco to Añasco Response to your request While our aim is to provide information and/or service whenever possible, in this instance we would need for you to personally file and request at BI Main Office. The said request must be filed personally to the BI Main Office by the owner of the information or the duly authorized representative (with Special Power of Attorney), which shall have pertinent documents attached, undergo assessment, evaluation and subject to the final approval/disapproval of the Bureau’s Commissioner.
