Hi... I would like to request for my Transcript of Records from my school Fukumoto International Training Institute located in Quezon avenue, which is now permanently closed. I studied 6 month Caregiver course and Nursing Aide which dated March 13, 2005 to September 14, 2005 for Caregiver and following the Nursing Aide course in the same year which I don't know the exact date.
