Hi, I would like to request for the following: My Transcript of Records for schoolyears 1997 - 2000 from Pilar High School, Sorsogon. (1st year, 2nd year - 4th year highschool) A copy of my highschool diploma My secondary school is far, I need to go to province to request the TOR and diploma. So, I'm hoping that academic transcripts have been forwarded to the Dep't of Education. Thank you for your assistance.

You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance: - https://www.deped.gov.ph/wp-content/uploads/2019/01/DO_s2016_54.pdf For further clarification, you may directly seek the assistance of the Schools Division Office near your area.
