Your Request You asked for Transcript of Records. Response to Request You may personally visit the Registrar’s Office from Monday to Friday 8:00 AM to 4:00 PM to request the needed documents. For your TOR request, proceed first to the Registrar's Office window, accomplish the clearance form and get the payment slip and pay the amount due at the Cashier's Office. Present the duly accomplished clearance form and official receipt to the Registrar window and get the claim stub. Bring 1 pc. documentary stamp. All requested documents are processed within 7 to 10 working days upon receipt of payment. An authorization letter, ID of the student and ID of the representative must be presented if the concerned student is not available to receive the requested document on the scheduled date of release.
