Commission on Higher Education(CHED)

Hi! I would like to request a copy of my transcript of record. I went to MFI Polytechnic Institute Inc. last 2014. The school is located in Ortigas Avenue, Pasig City and now I am currently residing back in my hometown. I contacted our school before in 2016, and was told that it will take time to request for my TOR since it was already sent to CHED and I no longer have a place to stay there.

Response to your request

To clarify, CHED does not release a transcript of record but only processes the Special Order (SO) number reflected on the said document as requested by HEIs.  As per Article XX section 97 of the Manual of Regulations for Private HEIs, "It shall be the duty of the higher education institution to release the school records of a student who has no outstanding property and/or financial obligations to the institution or is not under penalty of suspension or expulsion."  Furthermore, as cited in Article XXIII Section 110 of the manual, the HEI shall file an application for SO at least 60 calendar days before the end of the academic period and then CHED shall process their application and return to the HEI within 30 calendar days upon receipt of the application.
