This is in response to your request dated Feb. 2, 2024 seeking for Transaction Number to claim National ID card pursuant to Executive Order No. 2, s. 2016 on Freedom of Information in the Executive Branch.

In reference to your concern requesting for the transaction reference number (TRN) for lost transaction slip, please coordinate directly and provide the following details to our PhilSys Registry Office through email address [email protected]:
1) Name (Surname, Given name, Middle name)
2) Date of Birth (Month/Day/Year)
3) Sex
4) PhilSys Registration Center (RC) where you registered
Additionally, the Post Office will attempt to deliver your PhilID to the address you provided during your Step 2 registration. You may authorize an individual to receive your PhilID, or you may claim your PhilID at the Post Office. Please provide an authorization letter and a copy of your ID to your authorized representative.
