Thank you for your request dated Nov 30, 2021 01:53:27 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.
Your Request
You asked for Traditional Life and Variable Life License or Certificate for Nigel Atuel.
Response to Request
In relation to your request, please be advised that based on the records of this Commission, a traditional and variable life license were issued to you with validity from 2014 to 2016.
In this regard, please be informed that you may secure a Certificate of Agent’s License from this Commission through the submission of a formal request letter to the Licensing Division. The request letter should include the type of license being requested for certification, and the company being represented. You must also prepare a certification fee in the amount of Five Hundred Pesos (Php500.00)
Please note that this Commission can only issue a Certification for examination administered by this Commission.
Moreover, please be reminded of the following: 1) Setting of appointment thru the IC Appointment System which can be accessed through https://web.insurance.gov.ph/icare/login ; 2) Office hours of the Commission is from 9:00 A.M. to 4:00 P.M. only, and 3) Cut off time of our Cashier Section is until 2:00 P.M. only.

Your right to request a review
