I am writing to request my transcript of records from the University of the East Ramon Magsaysay Memorial Medical Center (UERMMMC) for the period of June 1999 to January 2000, when I was enrolled in the Bachelor of Science in Nursing (BSN) program. I also request my transcript of records from the AMA Computer Learning Center (AMACLC) Baliuag for the period of June 2000 to June 2002, when I completed a diploma in computer programming.
