Hi, May 2018 when we started the process of correcting my father's last name and birthday in his birth certificate but up to this day, we have not receive any feedback from PSA Bacuag, Surigao del Norte even though we have submitted the necessary docs. PSA Ref. No. 19CRS00- 138. - In that email, my concern was not addressed.

February 15, 2019 Dear Rexier Biong, Greetings! Thank you for your request dated Feb 15, 2019 04:11:47 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for The timeline/duration of the process of correcting last name and birth date in birth certificate.. Response to Request Your FOI request is approved and we are pleased to provide you details as follows: For a change of entry (e.g. name, sex, birthdate etc.) of the birth record of the individual, the petition shall be filed to the Civil Registration Office where the individual’s birth record was registered. Fees will be required for the petition of the correction of entry. Requirements such as school records, baptismal certificate, etc. (requirements will be provided by the Local Civil Registrar of the LGU) shall be prepared and submitted. The process is normally completed in three-month period. The petition and the required documents will be forwarded to PSA for validation and approval. Once approved, the PSA will return the said documents to the LCR for final approval and for provision of Certificate of Finality. The individual must submit the said approved documents for annotation to PSA- Central Outlet in Sta. Mesa, Manila City.
