In lieu of gathering data about my thesis topic entitled: PHILIPPINE STROKE CENTER: A SPECIALIZED HOSPITAL INTEGRATED WITH MEDICAL OUTPATIENT CARE FOR DIABETES, DYSLIPIDEMIA & HYPERTENSION, I respectfully request that may I kindly be allowed to acquire data and information which would be an integral part of our research undertaking, specifically:
•	Statistical Data of Stroke Cases in the Philippines by Region
o	No. of death due to stroke and stroke survival
o	Stroke Cases with Diabetes
o	Stroke Cases with Dyslipidemia
o	Stroke Cases with Hypertension

While our aim is to provide information whenever possible, in this instance this Office does not have the information specific to your request.
