Dear Honey Ann Penes,
I refer to my letter of Feb 02, 2023 10:54:21 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for The National Achievement Test 2018 Science 6 Result in Schools Division Office of Legazpi City.
To process your request, kindly submit the following documents:
a. Letter specifying the purpose of the request and the data being requested (year administration, scope, variables);
a. School ID to show proof that you are enrolled in a higher education institution;
B. Copy of your approved thesis/dissertation proposal signed by your Adviser;
c. Endorsement letter signed by the school Dean.
Please be informed that the Bureau of Education Assessment (BEA) can only release 10% of the actual number of examinees per year. Individual test results with names shall not be released unless the “data subject agrees to the collection and processing of personal information about and/or relating to him/her” per Section 3 of Republic Act 10173 or the Data Privacy Act of 2012.
Additionally, a corresponding amount shall be charged by BEA for the processing of the data request.

1. Describe the locale of our study.
2. Discuss the performance of SDO-Legazpi City in the recent National Achievement Test 2018 in Science 6.
The purpose of the information is to substantiate the literature of the study and to describe the reason why ICT integration should be highly considered in education.
Attached to this are the following:
1. School ID
2. Copy of our Research Proposal submitted for Defense.
