Thank you for your request dated Jan 14, 2021 05:20:46 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of DOLE-TABANG Application. OWWA Verification System shows that you applied online on January 12, 2021 for DOLE-TABANG and was evaluated by OWWA-RWO 5 as PENDING for submission of the following documentary proofs: (1) plane ticket of arrival to the Philippines, (2) CENOMAR, (3) Birth Certificate of dependent and (4) Proof of Loss of Employment on account of COVID-19.
