Good Day, I would like to request for changing of claimant for my Tabang Financial Assistance, I already have the transaction number from palawan but I cannot go out because of some health conditions that's why I'm requesting for changing of claimant or just bank transfer for safety purposes. April 13, 2021 Dear Mr. Tirso Roxas, Greetings! Thank you for your request dated Apr 08, 2021 03:57:17 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for an update of your DOLE-TABANG Application. OWWA Verification System shows that you applied online on February 3, 2021 and was evaluated by OWWA-RWO 4A as QUALIFIED on 03/31/2021. Your documents were forwarded to DOLE Regional Office IV-A on 04/11/2021 for the processing and release of your DOLE-TABANG Benefits. Any requests for changes of claimant must be done personally or by your duly authorized representative with the DOLE Regional Office IV-A at Calamba City or the nearest DOLE Provincial Office. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.
