I am requesting a copy of Student Permanent Record or Form 137. My name is Rosette Wheng A. Villalon and my birthdate is March 19, 1996. I attended Praise Emerald International School with the address of 121-41 Rotary Drive, Commonwealth Avenue, Quezon City, Metro Manila, Philippines during my 1st year and 2nd year of High School with the dates of 06/2008 to 04/2010. I also attended National College of Business and Arts – Fairview Campus with the address of Commonwealth Ave., cor. Regalado St., Fairview, Quezon City during my 3rd year and 4th year High School with dates of 06/2010 to 04/2012.

Your Request You asked for Student Permanent Record (Form 137) and High School Diploma. Response to Request Please be informed that the Department of Education-Central Office (DepEd-CO) does not keep learners’ school records. The original school records are issued once only, hence, you are advised to get certification from the school where you graduated and bring it to the Division Office for a certified copy. If you need this for your job application abroad, you need to bring the certification or school records to the Regional Office for authentication (DepEd Order No. 48, s. 2017 dated August 25, 2017). For further assistance, you may directly coordinate with the Schools Division Office (SDO) which has jurisdiction over the school.
