Dear Henrick Tan,
I refer to my letter of Mar 28, 2022 07:24:10 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Structural Plans of Public Hospitals.
Upon checking with the Bureau of Design, this Department, the only plans that we have are:
1. Philippine Orthopedic Center Hospital
2. Jose R. Reyes Memorial Medical Center
3. Tondo General Hospital & Medical Center
Which are still subject for verification whether they are architectural or structural plans.
Kindly provide us a copy of a formal letter addressed to the undersigned, Mr. Andro V. Santiago, DPWH-FOI Officer,([email protected] cc: [email protected]) with your company/school logo as its letterhead, duly signed by you and your supervisor/professor and a copy of your company/school identification card. Please specify the data being requested, the reason for the request and your contact details. We will proceed once above-mentioned requirements are provided.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jun 23, 2022 06:10:52 AM, your request will be closed.
