Hi! I would like to request for information regarding. 1.)Loss/Stolen Cellphone 2.)To search for loss/stolen cellphone using IMEI. I also want the device to be black listed so no one can use it.

July 22, 2019 Dear Michael Ian Amigleo, Greetings! Thank you for your request dated Jul 20, 2019 11:06:26 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request: You asked for Stolen Phone. Response to Request: In reference to the above request , may we refer you to visit/contact our Consumer Welfare and Protection Division (CWPD) for the Blocking of Lost/Stolen Mobile Phone/Unit. Our office is available from Monday-Friday 8:00AM - 5:00 PM. Or you may send your formal written complaint to the CWPD thru: [email protected] or call at 9213251 & 9267722. The following are the requirements: For Walk-In 1. Mobile Phone/Unit box indicating the (IMEI) or Official Receipt 2. Two (2) valid IDs (at least one government issued ID) For Mail, Fax and E-mail 1. Duly accomplished and notarized form (Affidavit of Ownership and Loss with Undertaking), available at NTC Website (ntc.gov.ph) 2. Scanned copy of the Mobile Phone/Unit box indicating the (IMEI) or copy of Official Receipt with IMEI number 3.
