Hello, ma'am/sir I have already requested from DICT and NBI and the DICT referred me to you, as you can see, I am trying really hard to get my phone back, because it has many important documents that my family and I share, this information could be bad to us, so I am asking in desparation , please help us, as I have tried everything, this agency might be the last one I can turn to for help, So i simply ask for your kindness Sincerely, John Vitalla

September 23, 2020 Dear john vitalla, Greetings! Thank you for your request dated Sep 22, 2020 03:07:10 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request: You asked for Stolen Phone. Response to Request: Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information. The following are the requirements: 1. Duly accomplished Request for Blocking Form 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 2.3 Copy of duly notarized Affidavit of Loss Note: The Affidavit of Lost must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative. Your right to request a review: If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to: GAMALIEL A.
