February 13, 2023 Dear Emma Santiago, Greetings! Thank you for your request dated Feb 02, 2023 11:16:51 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Status Update Covid Claim Compensation Application. Response to your request If you are an employee who suffered from a work-connected COVID, please file your EC sickness claim at the SSS branch nearest you (for private sector employees); or at the GSIS (for public sector employees). The forms are available at the SSS/GSIS/ECC website. EC Claims Requirements: 1. Certificate of Employment (indicate the last day of duty prior to contracting COVID-19. 2. RT-PCR or Swab Test Result (Positive) 3. Medical Certificate or Home Quarantine Clearance 4. 2 Valid IDs Forms (SS Form B300 Sickness Notification and SS Form B309 Accident Report) If you already have an approved EC sickness claim at SSS/GSIS, you can then file an EC cash assistance claim at the ECC. The EC CA application form is available in our FB page and the requirements are as follows: 1. Proof of Approved EC Claim Voucher from SSS/GSIS 2.
