Unfortunately, your query is a bit vague. We would like to clarify what is the 100 k incentive you are referring to in your request. It would also be helpful if you can specify a location or office that you will be receiving the incentive from.

The 100k incentive i was refering to was a incentive for felicidad fernandez who reached at the age of 100 years old and was already died last december 2018 i think. This was a project of dswd that if you reach 100 years old you recieve 100k incentive. We are at la union region 1.

Felicidad fernandez was our lola. I just want to know if her incentive was already released. who receives it and when it was released thank you

Your request

You requested for the following: (1) Status of 100 k incentive for Felicidad Fernandez; and (2) If already released who receive it and when it was receive.

We asked for clarification regarding your request because it is a bit vague. You replied that it was for the centenarian gift and that Felicidad Fernandez was your grandmother.

Response to your request

Unfortunately, the information you are requesting falls under the list of Exceptions to the Right of Access to Information enumerated in the Inventory of Exceptions to Right to Access to Information issued by the Office of the President (attached as ANNEX B to the FOI Manual of the DSWD). Specifically, it falls under:

a.	Information of a personal nature where disclosure would constitute a clearly unwarranted invasion of personal privacy, personal information or records, including sensitive personal information, birth records, school records, or medical or health records;

However, you can ask the Local Government of your Municipality in La Union that query. Further, once you have verified that it has not been released, you can be qualified to receive the same subject to the following provisions, pursuant to DSWD Memorandum Circular No. 6, series of 2018 on the Amendments to Memorandum Circular Nos. 4 and 11, series of 2017, entitled Guidelines and Supplemental Guidelines on the Implementation of RA 10868:
 
1.	The DSWD Field Offices shall release the centenarian’s gift, together with the posthumous plaque of recognition, to the nearest surviving relative of the deceased centenarian. In case the deceased centenarian was, at the time of his/her death, already abandoned, neglected and unattached, and he/she has no surviving relative, then no other person is qualified to receive his/her centenarian benefit.

2.	For verification/validation, the nearest surviving relative shall submit the following documents:
a.	Birth certificate, baptismal or Confirmation records certified by the parish church or other religious denomination, marriage certificate, old school record, old employment record and/or any other document that will prove to the deceased centenarian’s age eligibility at the time RA 10868 took effect, if any.
b.	Death certificate of the deceased centenarian.
c.	Copy of I.D. of the nearest surviving relative.
d.	Certificate of Live Birth of the nearest surviving relative and other relevant documents that would establish the relationship to the deceased centenarian.
e.	In case there are various surviving relatives (e.g. children), a SPECIAL POWER OF ATTORNEY (Annex 1) authorizing one of them to receive the centenarian gift in behalf of all of them. The nearest surviving relative should also execute a WARRANTY AND RELEASE FROM LIABILITY FORM (Annex 2).

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Secretary ROLANDO JOSELITO D. BAUTISTA at DSWD-Central Office, IBP Road Batasan Pambansa Complex, Constitution Hills, Quezon City. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
