Hello officers good day! I would like to ask for a SAP form so I can access for the cash assistance. Me and my wife went to the barangay hall and talked to some of the barangay officials outside since they won't let us in. Whenever we asked for some updates, they will get mad at us and force us to go home. I am an on call driver and the only breadwinner in our family. However, because of pandemic I am currently unemployed. I did not receive any cash assistance from the government. Please accept my request so my family can receive help. Thank you.

Your request You requested the following: (1) SAP Form from DSWD for Cash Assistance. You would like to ask for a SAP form so you could access for the cash assistance. Response to your request Unfortunately, this is not a valid Freedom of Information (FOI) Request. Thus, we are constrained to deny your request. However, you may contact the DSWD Social Amelioration Program Hotline Numbers at telephone number 16545 or by cellular phones through calling 09474822864 (Smart), 09162471194 (Globe) or 09329333251 (Sun) or texting 09189122813 or by e-mail at [email protected] to inquire about your concerns and/or report irregularities about the Social Amelioration Program brought about by the COVID-19 pandemic. According to MC 009, s. 2020 entitled Omnibus Guidelines in the Implementation of the Emergency Subsidy Program of the Department of Social Welfare and Development and is available at https://www.dswd.gov.ph/issuances/MCs/MC_2020-009.pdf, it is the barangay that chooses the SAP beneficiaries. Specifically: X. Role of the LGUs The LGU shall perform the following: xxx 2. Prepare the list of target beneficiaries/clients as stated above; xxx Thus, it is the barangay which determines on whether you could be given a SAC Form or not. It is also them who can possible add you to the list of the 2nd tranche beneficiaries. Further still, for other related concerns, you may wish to direct your concern to the DILG by submitting a request to them via this portal. You may also contact them by telephone at 8876-34-54 or fill-out the form located at their Public Assistance Center page at https://www.dilg.gov.ph/public-assistance-center.
