Hello good day.

I would like to have a sap form and a barcode so I can access the site given by the DSWD. My family did not received any cash assistance coming from the government. I am only a private tutor and I cannot teach any student since face to face interaction is prohibited. My mother went a several times  in our barangay but she all her efforts were unsuccessful. I did not receive any forms coming from the DSWD.

Please help us with our problem.

Yes, we do have some few questions with regards to our request. These are the following:
1. When can we have the SAP form or barcode?
2. Since the site given by the DSWD is already down, do we have to go to the barangay hall to present the given form or barcode?

Hello good day!

Last night my mother received the cash assistance, amounting of ₱16 000 through Gcash. We were so thankful that finally we have received the help coming from the government. 

I would like to CANCEL our request of SAP form.

You requested the following: (1)  SAP Form and Barcode so you could access the site given by the DSWD. Further, your mother went several times to your barangay to get the SAP form but all her efforts were unsuccessful.

Response to your request

Unfortunately, this is not a valid Freedom of Information (FOI) Request. Thus, we are constrained to deny your request.

However, you may contact the DSWD Social Amelioration Program Hotline Numbers at telephone  number 16545 or by cellular phones through calling 09474822864 (Smart), 09162471194 (Globe) or 09329333251 (Sun) or texting 09189122813 or by e-mail at [email protected] to inquire about your concerns and/or report irregularities about the Social Amelioration Program brought about by the COVID-19 pandemic.

According to MC 009, s. 2020 entitled Omnibus Guidelines in the Implementation of the Emergency Subsidy Program of the Department of Social Welfare and Development and is available at https://www.dswd.gov.ph/issuances/MCs/MC_2020-009.pdf, it is the barangay that chooses the SAP beneficiaries. Specifically:

X.	Role of the LGUs

	The LGU shall perform the following: xxx

		2.	Prepare the list of target beneficiaries/clients as stated above; xxx

Thus, it is the barangay which decides on whether you could be given a SAC Form or not. It is also them who can possible add you to the list of the 2nd tranche beneficiaries.

Your right to request a review
