Dear DOH,
I would like to request for a copy of the Statement of Appropriations, Allotments, Obligations, Disbursements, and Balances (SAAODB, FAR-1) for the three quarters so far of FY 2016, in open data or editable format (XLSX, XLS, CSV, or other but not PDF).
I note that the SAAODBs of DOH for Q1 and Q2 are already posted on your website via your Transparency Seal; however, these are not in a user-friendly format.
I also highly suggest that, henceforth, you post the open data file of the SAAODBs on your Transparency Seal alongside the PDF file.

Dear DoH,
The 15-working day time frame for responding to my request has already lapsed. Per Sec 12 of EO No. 2 s. 2016, "If the government office decides to deny the request, in whole or in part, it shall as soon as practicable, in any case within fifteen (15) working days from the receipt of the request, notify the requesting party the denial in writing. The notice shall clearly set forth the ground or grounds for denial and the circumstances on which the denial is based. Failure to notify the requesting party of the action taken on the request within the period herein stipulated shall be deemed a denial of the request for access to information."
I appreciate feedback if my request had indeed been denied, the grounds for such denial if it is indeed the case, so I may exercise my right to appeal my request (per Sec 13). Thank you.

I enclose a copy of the letter dated December 28, 2016 from Mr. Laureano Cruz, Director IV of DOH-Financial Management Service regarding the information you requested.
Your right to request a review
