Response to Request You may proceed to or call your school/ROTC unit/Reserve Center where you finished your ROTC program to request for your authenticated Certificate of Completion from the Admin and Personnel Officer of the unit or to retrieve your records and/or the Special/General Orders indicating your Name and AFP reservist serial number. Please prepare all pertinent documents and identification, including your TOR and valid government-issued IDs. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
