Good day! I would like to raise a concern regarding my vaccination record. I lost my vaccination card yesterday at the MOA Arena. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that I have a record. I was vaccinated in Sta. Rosa Laguna in Enchanted Kingdom. My first dose was on November 10, 2021 and the second dose was at December 02, 2021. As I stated above, I lost my vaccination card and I would like to know the steps to have new vaccine card if it's possible. Thank you very much!
