February 15, 2022 Dear Ms. Cristalyn Tugade, Greetings! Thank you for your request dated Feb 13, 2022 at 09:33:39 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch for a change of birth date in "dose 1" vaccine card to July 29 1989. Foremost, be correctly informed that the Overseas Workers Welfare Administration has NO JURISDICTION or MANDATE regarding COVID-19 Vaccine records. Hence, may we suggest to address your concern with the Bureau of Quarantine of the Department of Health for the correction of information on your vaccine card. Their hotline numbers are posted at the DOH Website. If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Administrator Hans Leo J. Cacdac at email: [email protected]. Your review request should explain why you are dissatisfied with this response and should be made within FIFTEEN (15) calendar days from the date you received this letter. We will complete the review and tell you the result within THIRTY (30) calendar days from the date we received your review request.
