I would like to request for information about Teofilo Macabasag, who was a native of Guiuan, Eastern Samar:
1. His position title while he served our country and people during World War II;
2. His job description during that era (i.e. WWII);
3. The name of his platoon or team that he worked with during WWII;
4. The specific place (town, village, etc.) where he performed his job;
5. The specific time or period of his employment (month, day, year) when he conducted his job or function during WWII;
6. And other information about Teofilo relative to his service as a WWII veteran.

Dear Teofhen Macabasag,
I refer to my letter of Jun 10, 2021, 08:45:24 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Requesting records and information of my grandfather as a veteran.
In order for us to work further on your request, we need more information from you regarding your query. Mr. Macabasag.
Your requests have been filed and it appears that Veteran Teofilo Macabasag is indeed registered as our pensioner. Following your requests for his records you must first provide the following details:
- Request Form (which can be secured of the VRMD Front-line Service) available at our PVAO website.
- Photocopy of 2 valid IDs
- and a letter addressed to MS. OLIVIA C. ALEJANDRINO, Acting Chief, VRMD (Veterans Records Management Division), stating your relationship with Veteran Macabasag and the reason for your request to acquire his records.
Once this is answered, Philippine Veterans Affairs Office, will undergo thorough research and will coordinate with you soon once it finished.
May we remind you that the records we have are dependent upon the submitted information given to our agency when they have applied for PVAO Pension.

Dear Teofhen,
I refer to my letter of Jun 24, 2021 about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Requesting records and information of my grandfather as a veteran. In particular, I had requested specific information for you to clarify upon in order for us to proceed with the request.
As we have not received a reply within the 60-day working period since the letter requesting for clarification was sent out, we regret to inform you that this request will be closed as of Sep 16, 2021.
You may wish to submit a new request describing the specific information you are looking for, and we will be able to consider it under the guidelines as stated in Executive Order No. 2.
Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to or visiting our central office. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
