Hi, we are sincerely requesting your kind office to help my cousin for her cremation & burial expenses. She died last Sept 3,2021 at the age of 21 due to covid. She was orphaned by her mother’s passing few months before her death and she has no one to help her on the financial needs for her burial. We have collected almost half of the needed payment already. I have the copy of her death certificate and certificate of indigency, and the promisory note from the funeral home, when needed. Thank you!

Thank you for your request dated Sep 23, 2021 04:20:15 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for Requesting Cremation & Burial assistance of Covid Patient. Response to your request We wish to inform you that the coverage of financial assistance under the PCSO Medical Assistance Program is for hospital confinement, chemotherapy and dialysis requests. Application for assistance shall depend upon the location of the health facility where the patient is being managed/treated. For facilities within Metro Manila, assistance is processed through online application at the PCSO Official Website. Please go to , find the E-Services tab and click on the NCR Online Application for Medical Assistance. For the Medical Assistance Program requirements, kindly go to this link https://www.pcso.gov.ph/ProgramsAndServices/CAD/MedicalAccessProgram.aspx. For those in the provinces, either proceed directly to the servicing PCSO Branch Office or coordinate first with the Medical Social Services or Billing Department of the facility, if applicable, to inquire on the specific process, as procedures vary between Branch Offices. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to . Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
