Madam/Sir; I am Barry L. Sebastian and I'm requesting a copy of my Form 137 from Don Pablo Lorenzo Memorial High School, Zamboanga City, where I graduated high school from SY: 2003-2004. I am requesting directly to the DepEd Office with the help of an online request since I already reside here in Bacoor City, Cavite. It would be appreciated if you would kindly furnish the said record because Form 137-A is a needed requirement for my job application to work abroad. I cannot proceed with my application without the record of my Form 137. Your prompt action would be highly appreciated

You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance: - https://www.deped.gov.ph/wp-content/uploads/2019/01/DO_s2016_54.pdf For further clarification, you may directly seek the assistance of the school where you graduated or the Schools Division Office near your area.
