Dear Ms. Divina, Thank you very much for accepting my request. I will look forward to your response. May I know if you need other information other than the IMEI? I am hoping that you could help me track my cellphone, and to know whom is the person who got it. That cellphone is brand new going one month this coming June.That's why I am very eager to track it. #NTC-380188884723 Thank you.

May 20, 2021 Dear Kath Platon, Greetings! Thank you for your request dated May 19, 2021 10:41:45 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request to track my lost cellphone. Response to Request Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information or email at [email protected] . The following are the requirements: 1. Duly accomplished and notarized AFFIDAVIT OF OWNERSHIP AND LOSS WITH UNDERTAKING [Form No. NTC 1-24] Note: The Affidavit of Loss must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1.
