Good day! I would like to make a request regarding on blocking my stolen phone since I cannot track it anymore. This is for security purposes and so that thieves can't also use or sell it. My friend has the same problem so she gave me also the IMEI number of her stolen phone. My phone's IMEI number is 867791040053973 (Oppo A3s) My friend's phone IMEI number is 862386047160809 (Realme 5i) We will be glad if you will grant our request regarding of this matter. June 1, 2021 Dear Russel Marie Basanez, Greetings! Thank you for your request dated May 31, 2021 02:24:38 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request to block stolen phone's IMEI. Response to Request Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we suggest you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information or email at [email protected] . The following are the requirements: 1. Duly accomplished and notarized AFFIDAVIT OF OWNERSHIP AND LOSS WITH UNDERTAKING [Form No. NTC 1-24] Note: The Affidavit of Loss must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1.
