EHi Good Evening po. Ipapa Block ko po yun phone ko nawala. Last september 1, 2020 . Di po ako makapag send sa email ng ntc. Yun [email protected] kasi full napo ang inbox. Ilan beses ko na po sinend un files ko dun. Umid id, affidavit of lost, un phone box po with imei and yun sim number then un receipt kopo ng phone. Wala naman po ako telephone para matawagan sila. I wish na matulungan niyo po ako na maipablock ito sa Ntc. Upang di narin iyon mapakinabangan ng iba. Thank you

Masyado naman po ako malayo kaya dipo ako makapunta sa Ntc. Pag sa email po full na daw po. September 9, 2020 Dear Maribel Cornejo, Greetings! Thank you for your request dated Sep 07, 2020 06:16:01 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request to block my IMEI(Stolen Phone). Response to Request Please be informed that the NTC has no capability to locate lost phone. In reference to the above request, may we refer you to file a request for the Blocking of Lost/Stolen Mobile Phone/Unit. You may file it at Consumer Welfare and Protection Division (CWPD) at National Telecommunications Commission BIR Road East Triangle Diliman Quezon City . Our office is available from Monday-Friday 8:00AM - 5:00 PM or call at 8-9213251 & 8-9267722 for further information. The following are the requirements: 1. Duly accomplished Request for Blocking Form 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 2.3 Copy of duly notarized Affidavit of Loss Note: The Affidavit of Lost must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
