H! I would like to request for the information regarding my lost phone. IMEI1 - 869789040378521 IMEI2 - 869789041078526 My phone's model is Redmi Note 7, space black in color. My first and last phone number before the phone got lost was 09175145857. I lost it this January 6 at Floridablanca, Pampanga. My email is [email protected]. If any documents and proof of ownership is needed, I would gladly email it. Thank you in advance for considering my request.

January 15, 2020 Dear Nicole Morcilla, Greetings! Thank you for your request dated January 15, 2020 9:31 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request: You asked for Request Regarding My Lost Phone. Response to Request: In reference to the above request , may we refer you to visit/contact our Consumer Welfare and Protection Division (CWPD) for the Blocking of Lost/Stolen Mobile Phone/Unit. Our office is available from Monday-Friday 8:00AM - 5:00 PM. Or you may send your formal written complaint to the CWPD thru: [email protected] or call at 8-9213251 & 8-9267722. The following are the requirements: 1. Duly accomplished Request for Blocking Form 2. Proof of Ownership, ANY of the following: 2.1 Copy of Official Receipt of the mobile phone 2.2 Box of the mobile phone with International Mobile Equipment Identity (IMEI) 2.3 Copy of duly notarized Affidavit of Loss Note: The Affidavit of Lost must indicate the IMEI of the mobile phone and/or the SIM to be blocked. 3. Copy of valid Identification 3.1 Any government-issued ID or Passport 3.2 For students, school ID 3.3 For cases when ID is not available, Birth Certificate OR NBI Clearance Supporting Documents for Representative(s) 1. Authorization letter duly signed by the applicant and valid ID of the authorized representative.
