Hello, Good day! My name is Joyce Nikka Palattao Zingapan. I am a graduate of the senior highschool back in 2019 at Polytechnic University of the Philippines in Sta. Mesa, Manila. I would like to request for a Certified Original Copy of my Form-137 as I don't have my original copy. I am requesting for original one because I am applying for Government Intership Program in DOLE NCR. I would be willing to pay any extra charges for this. Hope you can get back to me as soon as possible. Looking forward to hear from you. Thank you!

August 30, 2022 Dear Joyce Nikka Zingapan, Greetings! Thank you for your request dated Aug 10, 2022 12:05:22 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your Request You asked for Request of Form 137 & 138, and certificate of good moral character. Response to Request Your FOI request is approved. Kindly visit the PUP Online Document Request System (PUP ODRS) https://odrs.pup.edu.ph . Select your Registrar. Create an account then request TOR, pay and submit the requirements. There will be a status monitoring for your requested document. You will be informed if it is ready for pick-up. Upon pick-up, present client’s copy of application form together with authorization letter and ID, if claimant is immediate family member or Special Power of Attorney (SPA), if claimant is other than immediate family member. You may also email the Senior High School department at [email protected]. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Vice President Pascualito B. Gatan at [email protected]. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter.
