Philippine Overseas Employment Administration(POEA)

Hi. I would to request a travel history record for my lost passport. My agency asked me this for processing of my working visa in taiwan.

Question:
1. For number 2 requirements, what if i dont have copies of my arrival and departure since it's already lost. 
2. For number 4 requirements, Do I really need to get letter from POEA since purpose is for employment abroad?
